Causes

  • Vitamin B12 deficiency (subacute combined degeneration)
  • Nitrous oxide associated
  • Malnourishment, strict veganism, metformin use, HIV-associated, multiple myeloma-associated
  • Copper deficiency myelopathy
  • AIDS myelopathy
  • Spinal cord compression (eg from cervical spondylosis)
  • HTLV-1 associated myelopathy (tropical spastic paraparesis)

Clinical presentation

  • Impaired proprioception and vibration perception
  • Sensory ataxia with positive Romberg sign
  • Intact pain and temperature perception
  • Bladder atony may exist
